The Colorado Coalition for the Homeless (CCH) has been dedicated to addressing the healthcare needs of individuals experiencing homelessness in Denver since 1985. This commitment culminated in the establishment of the Stout Street Health Center, a leading Federally Qualified Health Center that opened its doors in 2014. Stout Street Health Center is specifically designed to provide comprehensive and accessible healthcare services to adults and children facing or at risk of homelessness.
Understanding the multifaceted challenges faced by this vulnerable population, Stout Street Health Center offers a wide range of integrated services under one roof. Patients can access medical and behavioral health care, substance use disorder treatment, dental, vision, and pharmacy services, along with crucial case management support. This integrated model significantly reduces barriers to care, ensuring that individuals can easily access the help they need. Through Stout Street Health Center and its network of five satellite locations, the Colorado Coalition for the Homeless serves over 14,000 patients annually, making a substantial impact on the health and well-being of Denver’s homeless community.
Services Offered at Stout Street Health Center
Stout Street Health Center and its satellite sites provide a comprehensive suite of healthcare services, including preventive and primary care to maintain wellness and address acute and chronic health issues. Dental services are available to ensure oral health, while ophthalmic and optometric care addresses vision needs. Recognizing the significant impact of mental health and substance use on the homeless population, the center also offers specialized mental health and substance use disorder treatment programs. Furthermore, pharmacy services are conveniently located on-site, and assistance with Medicaid enrollment is available to ensure financial access to healthcare. For individuals needing a safe space to recover after hospitalization, Stout Street Health Center also provides medical respite care.
Beyond immediate healthcare needs, the Colorado Coalition for the Homeless offers extensive support services to promote long-term stability and well-being. These include permanent supportive housing and affordable housing options, street outreach to connect with individuals in need, case management to navigate complex systems, benefits acquisition assistance, a children’s center, vocational services to gain employment skills, residential services, and rental assistance programs. CCH also actively engages in education and advocacy at the state level in Colorado, championing policies related to housing funding, renters’ rights, health and substance use services, and criminal justice reform, all aimed at supporting vulnerable populations. To date, the Coalition’s housing development efforts have resulted in over 1,900 housing units for individuals experiencing or at risk of homelessness.
